چکیده

Methods. A maximum of 20 volunteers were included to receive an ultrasound-guided sciatic nerve block with mepivacaine 1.5% and a starting volume of 0.2 ml mm cross-sectional nerve area. In cases of a complete sensory block, the volume was reduced by 0.02 ml mm cross-sectional nerve area until the first block failed. Thereafter, the volume of local anaesthetic was increased by 0.02 ml mm cross-sectional nerve area. After three cycles of successful/failed blocks, the ED99 volume of local anaesthetic could be calculated by a probability function. The influence of the volumes of local anaesthetics on sensory onset times and duration of sensory block was evaluated by linear regression.

A novel technique for learning to intubate with the lightwand.

Editor, To achieve extended anaesthesia and analgesia, a psoas compartment–sciatic nerve block (PCSNB) requires administration of large volumes of potentially cardiotoxic local anaesthetics. Due to the anatomic location of the lumbar plexus [1], mostly within muscle tissue, initial absorption of the administered local anaesthetics after a psoas compartment block could result in relatively high ...

Regional anaesthesia.

Sciatic nerve block is frequently used for anaesthesia or analgesia during orthopaedic foot surgery and there are several different approaches to the sciatic nerve. This report describes a new approach to the sciatic nerve using ultrasound. Local anesthetic was injected into the ‘subgluteal space’ under ultrasound guidance which was effective in producing sciatic nerve block in a small series o...

Serratus plane block: a novel ultrasound-guided thoracic wall nerve block.

We present a novel ultrasound-guided regional anaesthetic technique that may achieve complete paraesthesia of the hemithorax. This technique may be a viable alternative to current regional anaesthetic techniques such as thoracic paravertebral and central neuraxial blockade, which can be technically more challenging and have a higher potential side-effect profile. We performed the serratus block...

Determination of the EC50 of levobupivacaine for femoral and sciatic perineural infusion after total knee arthroplasty.

BACKGROUND Infusion of local anaesthetic through femoral and sciatic catheters is an accepted method of providing pain relief after knee arthroplasty. However, the minimum effective concentration of perineural local anaesthetics is not known. METHODS Twenty-four patients received femoral and sciatic perineural infusions of levobupivacaine in order to prevent pain relief after total knee arthr...

Does circumferential spread of local anaesthetic improve the success of peripheral nerve block?

BACKGROUND The relation between the pattern of local anaesthetic (LA) spread and the quality of peripheral nerve block is unclear. METHODS Twenty-one volunteers were randomized to receive a median nerve block with intended circumferential or intended non-circumferential spread of LA. Different predetermined volumes and needle placement techniques were used to produce the different patterns of...
